The EVSC Office of Health Services and Wellness highly recommends that students have a physical examination prior to entering Kindergarten, 6th and 9th grades. It is recommended that you make an appointment with your doctor and return the physical exam form to your school. Your doctor’s office will most likely have a physical exam form.
